Here are the copies for the 2020-21 physical packet for students that have an existing physical on file from 2019-20.  Updated physical info packet. 

If you have a current physical for the 2019-20 school year, the IHSAA will honor that physical for 2020-21.

The supplemental pages that accompany the physical will need to be completed for 2020-21
